PROBLEM PATRON REPORT

A Problem Patron Report should be filed via the Online Accident Reporting System found at http://oars.ehs.washington.edu/. (Instructions at https://www.ehs.washington.edu/ohsoars/index.shtm.) The report should be filed whenever a staff member becomes aware of a problem involving problem patrons. This report must be filed within 24 hours of the occurrence.

Include the following information in the report:

  • Patron name (if known)
  • Physical description
  • Information regarding your knowledge if this patron has caused problems for the Library in the past or not
  • Details of the incident including date, time and location of occurrence
  • Staff member(s) involved
  • Witness(es)
  • Description of the incident
  • Were the UW police contacted? Date of report, reported by phone, mail or in person and case number assigned.
