SERIOUS SECURITY INCIDENT REPORT

A report should be filed via the Online Accident Reporting System found at http://oars.ehs.washington.edu/. (Instructions at https://www.ehs.washington.edu/ohsoars/index.shtm.) The report should be filed whenever a staff member becomes aware of a problem involving serious security incidents. This report must be filed within 24 hours of the occurrence.

Include the following information in the report:

  • Full description of the incident, including names of responding UW police officers, any actions taken by the police and case number assigned.
  • Complainant name (if known) or physical description. Indicate if complainant is patron or staff.
  • Complainant address/phone number.
  • Details of incident.
  • Date and time and location of occurrence.
  • Staff member(s) and witness(es) involved.
