Instructions for Completing UWATS Code Request Form

Instructions:

  1. Download and complete the PDF form: UWATS (UW Alternative Toll Service) Code Request Form.
    1. Under Authorization Code select "Add".
    2. If you don't know which budget you'll be using leave it blank for Eshy Shahrazad to fill in.
    3. Enter your name under "Authorization Name".
    4. Check which features you want to use: Metro UWATS, UW Direct, International Calling or Calling Area Restriction.
      If you need help deciding what optional features you need, the UW Technology Service Center can be contacted at 221-5000.
  2. Print the form.
  3. Make a copy of the form for your records.
  4. Submit printed form to Eshy Shahrazad in Administrative Services, who will handle approvals, notify fiscal staff, copy for Administrative Records, and forward to UW Technology for processing.
  5. When your UWATS code arrives, it will be forwarded to you.

Policies:

  1. It is your responsibility to track calls made each month by either listing them on your calender or using the UWATS Log, in the event of an audit. The Libraries will periodically ask that you verify calls made on your access code number by sending the monthly report to you and requesting your signature.
  2. You need to ensure your code is secured and not left out where others can use it. If you lose your code or discover it is missing, let Eshy Shahrazad (206 543-3401) know, so the code can be canceled and a new one issued to you.

Monthly Reporting:

At the end of the month a bill will be passed around to those who made calls, to verify calls made the previous month.

Record Keeping:

You can either list them on your calendar or use the UWATS Log (PDF format -or- MS Word format) found on Staffweb in the Forms Toolkit, to check off your bill.

Security of the Code:

You need to ensure your code is secured and not left out where others can use it. If you discover a series of calls not made by yourself, please identify them and report them to Eshy Shahrazad in Administrative Services immediately. It may necessary to cancel your code and obtain a new one.

Reminder:

You are responsible for all the calls charged to your code and need to sign off on them each month. If you make relatively few call it's easy to note on the bill 206-543-3401 --> Interviews "xyz" candidate, 206-543-5531 --> "abc" vendor contacted, etc.
