Staff Change Request Form Instructions

For a new employee:

As soon as you have made a decision to hire an employee, please complete as many parts of the Staff Change Request form as you are able.
Under "Employment Status" select "NEW Employee"
If you don't have all their information, please have the new employee complete the Staff Information Update Form, which is located in the Staff Toolkit, when they arrive. Please also let your new person know that they can make updates to their information via the Staff Information Update Form as well.

For a departing employee

As soon as you have a termination date for an employee, please complete the Staff Change Request form.
Under "Employment Status" select "DEPARTING Employee"

To change or update information for an employee

Have the employee complete the Staff Information Update Form, which is located in the Staff Toolkit.
If you wish to update information for an employee yourself, please complete the appropriate section of the Staff Change Request form. Under "Employment Status" select "CURRENT Employee-Information Update"

Public information should be made available on HealthLlinks (http://healthlinks.washington.edu/hsl/).
Staff information should be made available on Staffweb (http://staffweb.hsl.washington.edu/).
