Supervisors:
If you have an employee who wants to take leave without pay, they
need to submit a memo to you in writing, have it approved by yourself
and send the original to Phyllis with a copy to Louise for our documentation
records. Authorization/approval will be returned by Phyllis. They'll
also need to put this information on the "Request for Leave
of Absence" form located on the back of their monthly timesheet.
Note: LWOP will extend employees in probationary status the same
number of days they are on LWOP.
The UW HR website
provides the following statement:
Requesting Leave of Absence Without Pay An employee may request
approval of a leave of absence without pay. Leave without pay normally
is requested in writing by the employee and must be approved by
the supervisor and/or department manager. Unpaid leave may be allowed
for time that an employee is absent for such reasons as disability,
parental leave, child care emergencies, and education.
An employee requesting a disability and/or parental leave of absence
is not required to use up all accrued paid leave balances prior
to taking leave without pay.
An employee taking an approved leave of absence without pay for
personal reasons (e.g., extended vacation) is to use appropriate
paid leave balances prior to taking unpaid leave.
